In this episode, Ben and I discuss his PhD thesis and his vast work experience in this area. Ben's PhD is focussed on seeking to understand how, in high-risk environments, we build up false assurance and false senses of confidence (fantasy planning) in our safety systems, plans, controls and practices which are not aligned to the actual or likely degree of harm/vulnerability.
